THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

1. Our Commitment to Your Privacy Voth Vitality LLC is required by law to maintain the privacy and security of your protected health information (PHI), to provide you with this Notice of Privacy Practices, and to follow the terms of the notice currently in effect. We are required to notify you promptly if a breach occurs that may have compromised the privacy or security of your information.

While Voth Vitality LLC is a cash-pay medical practice and does not participate in health insurance plans, we are committed to protecting your health information in accordance with applicable federal and state privacy laws.

2. How We May Use and Disclose Your Health Information We may use and disclose your health information for the following purposes:

Treatment: To provide, coordinate, or manage your healthcare. This may include sharing information with other healthcare providers involved in your care (e.g., your primary doctor).
Payment: To obtain payment for services. Voth Vitality LLC is a cash-pay practice and does not submit claims to insurance companies. Your PHI will be used to generate invoices and collect payment directly from you or your designated representative.
Healthcare Operations: For administrative, legal, and quality improvement activities necessary to operate the practice.
Other Permitted Uses: We may disclose your information without authorization as permitted by law for public health activities, health oversight, or in response to a court order.

3. Your Rights You have the right to:

Access: Inspect and obtain a copy of your medical records within 30 days of a written request.
Amend: Request corrections to your records if you believe information is incorrect.
Accounting of Disclosures: Receive a list of certain disclosures we have made of your PHI.
Request Restrictions: Ask us to limit how your information is shared. Because you pay for services out-of-pocket in full, we will honor requests not to share your treatment information with health plans.
Confidential Communications: Request that we contact you in a specific way (e.g., home vs. office).
Paper Copy: Receive a paper copy of this Notice at any time.
